  Provider: Primary Care Health Services, Community Leadership Development Institute, and Ursuline Senior Services

  Program Description: To improve resident’s overall quality of life, improve resident’s overall health and well being. Improve human services to older adult and disabled residents. Increase social and recreational activities. Expand the capacity of resident councils, and improve linkages between residents and available community resources.
• Health Awareness & Screening for Seniors
  Provider: North Side Christian Health Center
  Location: 1014 Sheffield St., 1st Fl.
  Program Description: Provides residents with monthly blood pressure screening, glucose testing, medication review, health education literature and flu shots. Program held fourth Tuesday of the month, 10:00 a.m.-12:00 p.m.
• Mobile Post Office
  Provider: U.S. Postal Service
  Location: 1014 Sheffield Ave. (Rear of the high rise)
  Program Description: Full service post office for residents.
• Three Rivers Senior Center Lutheran Service Society
  Provider: Lutheran Service Society
  Location: 1014 Sheffield Ave. 1st Fl.
  Program Description: Provides a comprehensive continuum of services including: Senior Community Centers, Meals on Wheels, In-Home Services, Care Management, Protective Services, and Nutrition. On site programs are free for residents. Programs include: dinners, bingo, and social service casework. The center is open Mon.-Fri. 9:00 a.m. - 1:00 p.m.

From downtown, take the 9th Street Bridge to the North Side. Go under the overpass. Make a right (the only way to turn). Then an immediate left onto Cedar Avenue. Turn left onto North Avenue. Follow North Avenue to Allegheny Avenue and turn right. Turn left onto Sheffield Street. The entrance is on Sheffield Street.
